The chest for the item is usually right next to the quest-giver

I got the same Bug here. Cant finish the quest, so i'm stucked with this quest on my jornal, prevent news one to take his place.
By the way this bug is happening in a lot of accounts!

Also second day of the season so the xp means I can't get grave of venom on the same day as others.

I did the puzzle for the chest but after that there is no item and you can just talk to the NPC who requires the item to finish the commission
