Can confirm, I've run almost 400 bosses at this point and have only seen this a handful of times up until now. Doing these runs with Death it was happening back to back repeatedly which I've never seen before.

I had same problem with my friend. And someone told me that drop is 100% so its IMPOSIBLE that i took nothing. And maybe someone lying in my team...
Btw. Restart game everyone from team can help , problem that this same problem come after few bosses.
