As much as I like the game. I get pretty ticked off at the amount of times some enemy who I had no chance to react to either gets a cheap hit on me or a down. I like the idea of being more cautious and not being able to hear everything. And for that, it adds a layer of horror. But from a gameplay perspective, it sucks. Alot. Whether it's a bug or a feature, maybe try and find some kind of balance between the two. Like leaning against walls to hear through them a bit, or a quieter sound different from the sounds they usually make, kinda like a terror radius to an enemy in a small area, quiet enough to miss if you're not playing careful. But loud enough to be fair to players with an air of caution

i agree after playing escapemode in metro with 400 cells for 4 hours and my brain becoming numb from trying to filter noises to not miss either the butcher or one of the 2 creeping shamblers 
Thirded, I have not only have personally ran into silent butcher several times, I even see experienced players get screwed over by a silent butcher, on the Metro which meant to be the starter area.
