This has happened a couple times now, both on bone zone. After killing the drowner a static water graphic remains, and going under the 'water line' (like where that purple chest is) will damage you as if you were underwater. The only consistency I can think of beyond the level was that I was using OtC characters with the 'hail from the future' darkana. Sorry for the bad screenshot!

bump from 1.13.113, this is happening because of the instakill effects from Sonic Whip, Jewel Gun and Sanctuary
it could be that the same effect is also causing frozen Stalkers to not die sometimes, where they get hit by an effect that should have killed them, drop a chest, but they keep following you once the freeze end till a minute pass
