hard to believe or really strange... do you have some EMS-Logs related to the PSU failure? Or did an power outage occur for the rack/psu lines etc.) Maybe something in the front end reqired a pathfailover (e.g. a switch that was powered off )?

Yes it was a maintenance activity and after psu failure LIF gets failed too
