Based on some posts I saw about 9.12.1px, and not sure of 9.12.1p4 as the latest, there have been some issues.
What would be your recommendation?

Love the 9.12.1p4. but also depends on your complete landscape and integrations. Make sure the whole stack is supported by 9.12. Also when using Harvest/NaBox some additional rights might be needed.

9.12 has FlexGroup rebalance. If you have FGs I'd recommend it. P4 has a lot of fixes now that are making 9.12 fairly stable.
