Anyone able to help me work out the augments/gear score? Like... ideally how to max out everything would be nice. I've achieved everything including T4 but really struggling on how to get the GS up. To note, I have 500+ hours into this game but it's just not clicking for me and I'm tired of trial and error here.

After you reach 100 on your gathering tools, gear score becomes irrelevant and then it's a matter of what kind of playstyle you want to build for.

I'm stuck on most items being just barely under 100 (ex: pickaxe is stuck at 98)

Yeah, for that you should probably have your mystics unlocked and use some t4 ore to craft them. With it all charmed, enchanted and infused, it should put you a little over the 100 mark.
Alternatively, if you have access to the Watch, you could get someone to run you through the forest and desert vaults for t5 wood and metald
