I get a white square (see attached image) and cannot fire off my ult. I have tried leftclicking (nothing happens), rightclicking (ult cancels), pressing ult again (gives me the popup that its on cooldown).

Additional information: I have the ultimate bound to 5 (not numpad, just regular 5). When I rebind it to R, it works. When I rebind it to 1, it works. My ability 4 is usually on 1 and has always worked. Ability 4 on 5 also works, just the ultimate doesn't.

Do you happen to have 5 bound to an emote at the same time? Cuz I couldnt ult when I have it bound to an emote at the same time

Unbind emote, it will work.

There is nothing bound to 5 for me, I checked the keybinds. Is 5 hardcoded as an emote keybind? I also tested 4, which also does not work for ult either for some reason. Ability 4 works on both button 4 and 5 though.
