Around medium range or somewhere around that, I shoot directly at them and it looks like they took damage, but then they heal very fast in a second.

this happens to me sometimes, at all ranges, at around 20 ping, and using a sniper.
the bullet also goes through the other player sometimes
at first i thought i was just too bad to hit my shots, but one enemy stayed in the laser for the entire duration of it and didnt take any damage.
im pretty sure it can happen at any part of the body, but it might be the gap in between the head and the body
