I just looked it up again and the screenshot attached I believe is what I need. Just to be sure, here are my two answers:

To charge a card immediately and make sure that additional charge for usage shows on the invoice as charged and that usage line item is discounted from the invoice as already paid.

Yes. They pay for the yearly subscription, then let's say 3 months in, they breach their free usage tier, we update the usage, and charge immediately.

Yes I think the Usage threshold settings should allow you to perform this action. I would still recommend testing this out with some test Customers and perhaps try out our Test Clocks feature.

But for usage you could just use the settings and update the usage to above the threshold
