Usually it is due to unstable connection or maybe something fishy with payment gateways (which isn't very often though)
It'd be helpful to know if there's a pattern to look deeper into this

We have 6 customers with very similar issues, one or two have gotten back to us to say they didn't see the modal appear hence why they are failing SCA

So these 3DS issues can definitely be tricky. The best thing to do is to ask a customer to record a video where they have their developer console open to catch any errors, then go through the flow and record it and provide the associated video and SetupIntent or PaymentIntent to our Suppor team.
