Okay I think you can safely ignore that. I cannot find anything in our code base regarding requires_action in relation to Refunds. In general this is used to handle 3DS authentication for card payments.
Perhaps a user may be required by their bank to authenticate the refund back to a card but I haven't seen/heard of that.
