We aren't receiving any more invoice.upcoming events. Does the 3-day setting only apply to new subscriptions?

Do you have an example subscription ID that isn't generating these events that I can take a look at?

That's the thing, we should be receiving several events on that webhook

But none are showing up now, after we changed the setting from 30 days -> 3 days

If this setting is changed, does it only apply to new subscriptions, or all of them?

Ah - that's the issue. When you change that setting it applies to all new subscriptions immediately, but won't apply to past subscriptions until they've been renewed

And there's no way to set all existing subscriptions to use the 3-day setting for invoice.upcoming?

I believe the only way would be to reset the billing cycle anchor for all those subscriptions (which I wouldn't really recommend)
