Ok, here's a payment for which (it appears) we never received a webhook: https://dashboard.stripe.com/payments/pi_3JUHbeK8IruJbDfX07P2fQXz

if you look at the Events for either of these in the Stripe dashboard, i am not actually getting any data on the webhook attempts, so it's hard to say anything for sure
but i've been digging on this issue for quite some time and i've not really been able to reach any other conclusion than that is rare circumstances, we literally never got the webhook
Let me take a look! it's expected that the events don't appear in the dashboard since it's been more than 30 days, but let me see what we have in our logs

(the lack of webhook data there is not the reasoning for my hypothesis, i should clarify)
Okay, so from our logs I do see that it both the Payment Intents you sent over did generate checkout.session.completed webhook events that were sent to the same set of webhook endpoints and were all responded to with 200
dang, really...
thanks for checking into that. i guess i have to conclude that the issue lies somewhere on our end then
what's so incredibly weird though, is that in almost every case where we experience this, that same user will try to checkout again and it works the second time...
anyways, if you confirmed the webhook was indeed sent, i should stop bothering you
