The process we go through is collect info from a form. Then create a bank account token, then we create an account token, then we create person tokens for up to 4 people associated to the account. Then I believe we create the custom account using the token and info provided subsequently adding person and external account (bank)

Ah, I think I see the issue - when you created this account I believe you used the test ID number 000000000 (or 0000 if you just used a partial ID number) which will always pass verification. Using this successful ID number overrides passing in the failed ID image

Yes, use 111111111 or don't pass in an ID at all
