This file upload was made using a Test mode secret key so its not available in Live mode

I guess the web app used a stale session from test mode.
I had to logout make it work.

It's weird that a Live mode referenced a test file somehow in the web ui, isn't it?

i don't know what web UI you are referring to
If you are using the Stripe Dashboard, you can be interacting with it in either Test or Live mode. Perhaps there was some issue with caching?
