ok, your new phase doesn't have an end date, so by default it runs forever with the schedule attached
you have a few choices
you can either release the schedule manually via the API
or you can give you final phase an end date / termination condition
personally, i like to use a simple iterations: 1 to have the schedule stay attached for the first billing cycle, then it ends & releases (based on end_behavior: release).
But you can also set an explicit end_date if you prefer, a short time afte rthe start date, to force the release to happen sooner

I want that new plan to stay on the subscription
yep, when the schedule is released the subscription just continues with whatever price/items were in place at that point
