Your Dashboard settings say to void the invoice and leave the Subscription as 'unpaid' if payment fails: https://dashboard.stripe.com/test/settings/billing/automatic

yes, two questions

how did the subscription get created in the first place when payment was not successful or done.
the subscription got expired instead on unpaid. why?

The Subscription got created because that's how Stripe does things - it creates the Subscription then attempts to pay it.
