And are you using the client secret to render Payment Element or the deferred approach?

Gotcha and do you update the PaymentIntent's amount after rendering Payment Element?
Yes
Gotcha then when you do that before you have your submit handler called on your frontend you need to call elements.fetchUpdates(): https://docs.stripe.com/js/elements_object/fetch_updates
That will then reflect the updated amount in the Apple Pay modal
