Hello there
The Return URL is only hit when you use a payment method that requires a full page redirect.
This is oftentimes bank-based redirects: https://docs.stripe.com/payments/bank-redirects
If you are only supporting Card payments then you will never have the Return URL hit as there is never a redirect of this nature.

Ok, and if we get the redirect with an "open" status, I just all create-checkout-session again right? I should never have to store the secret in a DB on my side?

Correct, you should not need to store the client secret.
You basically fetch a new one any time you want to render Checkout
