Hello! You can add metadata to the Refund when you create it via the API to indicate the reason and other details: https://docs.stripe.com/api/refunds/create#create_refund-metadata
There is also a somewhat limited reason parameter you can also set, but it only takes a few values that don't sound like they would be relevant for your use case: https://docs.stripe.com/api/refunds/create#create_refund-reason
