You can't I don't believe. The new entitlement is applied at the start of the next billing period regardless of API/Dashboard
I'd imagine you'd need to handle the entitlement for those existing subscriptions internally somehow

Hm, I see:/ Doesn't that make the Entitlements API kind of pointless, or am I generally misunderstanding what Entitlements are intended for?

I don't think they're pointless no, it's just a feature gap right now that is not supported. I've shared your feedback
