Hi, that card require 3DS authentication. Without much details, it looks like you're not handling next_action, https://docs.stripe.com/payments/paymentintents/lifecycle. I do not have much expertise with Lalavel but that seems to be the case. As a next step, you'd want to ensure that you look at the status of the payment intent from the subscription, and complete next_action

Well this is whats confusing, as when I use the cardElement with the Stripe JS SDK and that card number (3155), the 3DS pop-up appears before the Setup Intent is confirmed.

The only way I can complete the action is by manually (and via email when in Prod) is to confirm the payment via the invoice.

I thought by using the Payment Method returned by the Setup Intent after confirming the 3DS prompt would allow me to create an "off-session" subscription

ok, and was that the intended behavior of that card or am i not doing something right. I know if i use the card ending in "4242" then the subscription is setup as expected

since you're setting up the payment method for off_session usage, it should just work.
