Stripe can, as far as I know. But that isn't an area we are experts on at this server.

The best place to get confirmation of how Connect Accounts get disabled would be reaching out to Support here: https://support.stripe.com/contact
And you can confirm that the only way to know if a connected account is rejected using the API is by reading "rejected" in the requirements.disable_reason ?
That will tell you the ultimate reason. For more specifics when the reasion is something like rejected.incomplete_verification you can review the past_due requirements to identify what verifications were missed
