Okay we're not finding much here. It looks like you have connection issues and the Reader is losing the internet connection a lot throughout the day and we think that's likely causing the issues where the payments just never complete.
but why would the terminal show successful?
I don't think it would but I'm not sure what you call "show successful" exactly.
the terminal would show the Green circle with the checkmark at the end of a successful tap

As far as we can tell it really is what I explained. You lose the connection and the PaymentIntent confirmation never happens/succeeds.

Taking over as my colleague had to go. Unfortunately, it seems that we show the green check mark on the reader screen when the payment method information has been successfully captured not when the Payment Intent has been confirmed.
It appears that between the two operations the network connection was dropped.
