I think you need to use a specific test card for this

Yes, we have tried those, note the link provided above, but those cards do not result in insufficient_funds error.
The guess is that, if connected account has insufficient funds, that refund should fail, is that correct?
Typically, the refund might still go through and connected account will carry a negative balance.
Stripe would attempt to recover funds by debiting connected account's attached bank account. https://docs.stripe.com/connect/account-balances#accounting-for-negative-balances
