We are not familiar with Magento here and I would suggest writting to Support

Generally requires_action means you would need to let your customer authenticate 3DS

#

and requires_confirmation means it still need confirm

it seems to be intermittent though, so sometimes I can checkout and other times it has an issue

It's triggered by each of the issuer bank. They decided to request 3DS

#

Your system should be able to handle it generally
