There is no way to set this via the API, but you can track this yourself and then cancel the PaymentIntent using https://stripe.com/docs/api/payment_intents/cancel

What if a malicious user uses a payment intent they generated from our app, and confirm it many times using the JS library directly (card testing) ? How can we track those confirmation attempts and block them ?

You could use Radar Rules for that. You can see a bunch of different options via https://stripe.com/docs/radar/rules/supported-attributes in terms of limitng the amount of charges
