I see a $10 payment on sub_1O5YleL0N6ZBv2bERtUZsSdS, but I don't see a coupon on that subscription. For the other three subscriptions I see a single $0 invoice, so it looks like the coupon worked properly on those

That is the same newly registered user who has just subscribed with the coupon, and the result was at least four active subscriptions, and one of them was a 10$ charge.
Gotcha, so to be clear, what I was looking at was the specific charges on each of those subscriptions. You can see them by going to the page for each of those subscriptions:

Is it the $96 payment that you are trying to look in to? Or how so many subscriptions were created?
Well, we use Stripe.js in the front-end, and the payment intents in the backend are optionally created with the coupon. We need to create a payment intent every time a user chooses the plan monthly or yearly subscription to refresh the prices presented to the end user. So, you can ignore the yearly $96 sub, because the idea was to give a user a monthly subscription with the first 6 months free. And after 6 months, it should start the normal charging cycle. I'm wondering why a single payment transaction has created four active subscriptions, one with the $10 charge.
From what I can see, there were four separate API calls to make these subscriptions and they came about ~10s apart from each other. The three subscriptions that had free trials had their first invoice immediately "paid" because it was only $0. For the subscription without a trial it looks like the user manually paid the invoice
