Is there a scenario where, after a successful payment, Afterpay won't redirect back to the URL we provide for returning the patient to our site?

I don't believe that should happen unless there's a breakage somewhere on Afterpay's site or your own server on redirect

How exactly are you integrated? Are you using Checkout or PaymentElement or something else?
