Ok, this setup is unsuccessful because the customer failed to complete the 3DS authentication. You can find the info from the setupIntent's last_setup_error

but customer personally did multiple 3DS authorizations with the same debit card and bank and never got this error.

I don't know how your customer performed the authentication, but at least they didn't complete for this one.

Did you customer face any difficulty or challenges when authenticating 3DS requested by this setupIntent?

yes when first time customer added 3ds card , he was redirected to error page. But on second try he redirected to authentication page

yes there was no issue from bank or card issuer.

can you please provide more insight why 3ds authentication failed. is there any issue from stripe side?

The 3DS authentication is provided by the card issuer, not Stripe. Based on the image that you shared, the TEXT_NOT_FOUND text doesn't look right to me, so there's a possibility that the 3DS authentication page is not working as expected.
