Yes, in this case it happened because you created this subscription with a trial period (for 14 days) with the setting to pause the subscription at the end of the trial is no payment method is set up to pay the invoice after the trial

This was initially alarming but it seems the creation of this subscription itself was outside of normal procedure

If that's not your normal subscription creation flow, then yea possibly! But this is normal expected behaviour based on how this was created.
