I may be mistaken, after further investigation, it appears that one of the confirm attempts resulted in "requires_action" status.
Generally for individual payment investigations I'd suggest working with support (and may here, still), but can I take a look at an example?

Typically this happens when you create a new payment intent instead of re-using a single one
In which case it is possible to accidentally double charge
