we have just implemented it and trying to test to see what the customer says, and if they are likely to dispute it thinking that there are two charges

do they see that as pending and then at end of order when it's captured and closed do they just see the full amount or do they see two charges

No, you would have to have a live charge to see what it will officially look like on their bank statement. They should see a pending authorization, then a charge. Once the charge goes through, the authorization should drop off such that there is usually only one transaction appearing at a time

is there a card to test with where the auth fails after initial hold when insufficient balance later

ie on auth it goes thru but on capture there are insuffic funds
