No you'll need to be approved first. I can't say how long that will take unfortunately
can I achieve the same thing with custom portal configurations (or at least very similar)? It looks to me that I can create a portal configuration for each of the predefined flows (like only allow canceling subscription, or updating it), and use that instead.

a follow up question. I'm currently fully evaluating the customer portal, but it looks that some requirements cannot really be implemented with it. On the other hand, I don't want to use Elements and ask for people to populate payment details on my site. Is there something in between? I don't mind to have some code on my side. Maybe a solution where users click upgrade/downgrade on my site, I update the subscription through an API, and somehow redirect them to Stripe to pay the generated invoice.
All you want them to do is to be able to upgrade/downgrade? If so, then yeah a button is all you need.

can they update their payment methods through that hosted invoice page? Like not only pay that invoice with a new card, but have the ability to mark that card as the default for all future payments
