The coupon/discount is applied on the whole invoice/subscription, not on a particular item of it. You can compute the target amount and create a coupon with a fixed amount that represents 25% off Item A and 100% off Item B.

If we do that, what happens when the user changes their subscription to remove item B themselves via the Customer Portal in 3 months time, they will be left with too much discount i assume?

So that's not really a viable solution then
