That's not correct. The bank can request at any time the user to go though 3DS.

So you need to handle that possiblity on your end. Which is to ask the user to come back to your website/app, and then re-confirm the PaymentIntent to start the 3DS flow.

Yes. I do do that. I have users that just don't bother doing it though. I only tell them payment failed so perhaps I should give them more information.

But good to know it can fail because of 3DS too.

Yes it's not ideal, but that's the only option: ask people to come back on yoru website/app and re-confirm the PaymentIntent to trigger the 3DS flow.

if I put a hold on funds instead would that trigger 3ds there and then?

When you put a hold, it would trigger 3DS when you confirm the PaymentIntent. Later when you capture the funds there would be no 3DS.
