Payment Element, I'm not sure about the second question, I'm trying to track down other payments that exhibit similar behavior

I haven't been able to reproduce or see it myself, but someone in my organization reported it so I'm trying to get them to track me down more info

Sometimes bank error codes aren't so straightforward. For example sometimes a CVC check can fail if the expiry is wrong, or sometimes you are actually declined for something else but the bank sends a CVC error instead of a more informative one

In this case the user can reach out to their bank to ask about the decline
