Hello! I'm not sure I understand your question. The screenshot shows what appears to be a custom form, not something Stripe built. If that form is giving you an error you need to talk to the people who built the form about the error you're seeing and where it's coming from.
My developers built the form based on Stripe data requirements. They have tested the form as it collects and passes data to Stripe as designed - for people with personal bank accounts. Do you know if there are different fields required for business bank accounts? Perhaps "Business Name, Business Address, account contact, etc." Thanks.
