This shouldn't be happening and the card issuer likely gave that response because the card was flagged for another reason (this is me speaking as a former card issuer support person). If you have successful payments that are occurring without bringing the customer back on-session, then this is probably happening on the card issuer's side.

  1. Is it even possible to setup a payment method for future off session payments that does not initially require 3D Secure, and then somehow require 3D Secure for subsequent payments?
    No. The customer has to be on-session to authenticate for 3DS
