If this was in test mode it may be because the test mode environment was overloaded at the time. It doesn't happen too often but it does happen

Hey apologies, the server got really busy all of a sudden. I haven't been able to look in to that subscription yet, I will ask a colleague to look in to it, we should be able to get back to you shortly

So it was from a temporary disruption in our test environment. This code should properly create subscriptions that immediately create their invoices in future
