No that's not possible. What's the usecase for having duplicates? Why not just alter quantity?

I figured. It's because the size of the item isn't attached to the product ID. So a customer could add a Large t-shirt to their cart and the same t-shirt to their cart in size Small. Both t-shirts have the same product ID. But they show different sizes in the cart table.
