the promotion code's restrictions.first_time_transactions is related to the promotion code, if the duration of the coupon is forever then it remains forever. You can create multiple promotion code for that coupon if you restrict one promotion code

Yes it will work forever
does promotion code's restrictions.first_time_transactions has no affect?
that restriction has effect only on the promotion code, not on the coupon itself. In case the promotion code is restricted, you can create another one to the coupon
