Is micro-deposit verification happening in all cases or in some cases?

Only in some cases! We know skip is enabled because it's not requiring micro deposits for payment methods created directly in the dashboard, its only when we try to create them via the above process that is requiring them. Stripe support has confirmed skip has been enabled as well

First example was one that required microdeposits, second one didn't

I believe this is expected behavior, as instant/skip verification uses microdeposit verification as a fallback for when the customer's bank doesn't accept instant verification or if instant verification fails

Gotcha, is there any way to confirm if it is the customer's bank not accepting the instant verification? Since it's failing consistently when adding the PMs one way vs the other it seems like an issue in our code

It doesn't look like there is a way to see if it was/wasn't supported for a given bank account.

Does your test app function as expected using the test account numbers alongside instant verifications?

Yep everything works as expected in test mode, but I don't think the test bank accounts ever ask for microdeposits in the first place? I've always seen the test bank accounts verify instantly

I think this needs a bit of a more asynchronous deep-dive on our end. Are you able to open a support thread and mention we talked on Discord? Once you create one I'll grab it and me or someone on my team will get back to you via email. I just don't think this is a straightforward issue with a response I can give over chat.
