I had my controller (normal PS5) plugged into the wall charger. It was fully charged, but suddenly turned off mid-game. I pressed the PS button to turn it back on, but it didn’t connect and was just flashing blue. I had to go into the app, switch it to native, then back to DualShock 4 emulation, and then turned the controller back on—only then did it connect. Are there any known issues you guys could recommend I look into

Apart from advising you to delete your controller from windows, reboot and then pair it again to start again on a clean slate, no idea what could have happened there.. Maybe your Bluetooth radio (from your PC) failed at some point and caused the controller to disconnect which turns it off, regardless of whether it's charging or not.
