Win key + R and then type %appdata%\.minecraft\crash-reports
mac: ~/Library/Application Support/minecraft

Possible Solutions:

OptiFine issue, seems your pack has broken connected textures. Either use a different pack or disable connected textures. (esc > options > video settings > quality and set connected textures to off)

Recommendations:

The version of Patcher you're using is outdated (1.6), make sure to update it at https://sk1er.club/mods/patcher
