any idea how to fix this if possible?

all the shelmets were obtained after you installed 1.0.3-RC1?

because unless the item dropped on the ground due to a full inv it should be accurate

it used to require you to open your inventory

as for inquis either your server REALLLLLLLLLLY SLOWO or you were more than 5.5 blocks away from it
