Possible Solutions:

Update Optifine to one of the latest versions, M6 if on macOS, M5 if not

Recommendations:

Most of the time you don't need more than 2GB RAM allocated (maybe 3-4GB if you use skyblock mods). You may be able to reduce the amount of RAM allocated from 6G to 2G or 3G
Update BlockOverlay https://hypixel.net/threads/forge-1-8-9-block-overlay-v4-0-3.1417995/, the version you have is extremely outdated (v2).
Update PerspectiveMod at https://inv.wtf/djperspective.
Update ToggleChat, https://no.boomy.wtf/?mod=togglechat&name=ToggleChat&version=3.0.39, the version you're using is outdated (2.1.0).
