Possible Solutions:

• Known issue with Levelhead, you'll need to remove it to launch the game. You may be able to put it back afterwards but it's not guaranteed to work.
• Update Optifine to one of the latest versions, M6 if on macOS, M5 if not

Recommendations:

• Update BlockOverlay https://hypixel.net/threads/forge-1-8-9-block-overlay-v4-0-3.1417995/, the version you have is extremely outdated (v2).
• Update PerspectiveMod at https://inv.wtf/djperspective.
