Possible Solutions:

Install/update Patcher to resolve this issue, or disable connected textures in video settings.

Recommendations:

LabyMod causes many issues with other mods. Try removing it and check if any issue you have is fixed.
Update Level Head from 7.3.0 to 8.2.3
Update Patcher from 1.6.1 to 1.8.9

Are you using 1.8.9 or 1.21.5?
